Production of Medical Records for Transfer of Care or Personal Use, above and beyond copies of laboratory testing. $0.75 per page for the first 25 pages, then $0.50 per page for the next 25 pages and $0.25 per page thereafter. Minimum retrieval fee of $10. Records will not be withheld for non-payment of these charges, but the unpaid fees for medical records account may be turned over for collection.
